1How do I make an appointment?
Please call our office Monday-Friday between 9:00 am and 5:00 pm or click here.
2What is the cancellation or rescheduling policy?
Please call us as soon as possible. This will allow another patient to use your appointment time. We request cancellation 24 hours prior to your scheduled appointment to avoid a no-show fee of $100.
3Do you offer same day appointments?
Same-day appointments are available for those patients who require urgent medical attention. If you require medical attention after regular office hours, a health care provider is on call at all times and can be reached by calling our office.
If you do not have a medical emergency, please do not call the emergency number. Call the office during normal business hours.
If you are experiencing a serious or life-threatening emergency, dial 911.
4Can I have access to my medical records?
Your medical records are available to you or another medical provider. You will need to sign a medical records release authorization that you must submit to our office before we can release your medical records. We will then provide a copy of your records for a nominal labor/copy fee.
5When is payment due?
Payment/Co-payments are due at the time of service. During your first visit, you will be asked to fill out and sign a credit card authorization that will be used to cover balances of charges not paid or covered by your insurance.
6Do you fill out disability or other forms?
We must have a signed medical record release form prior to filling out any forms that contain any protected medical information. In order to apply for disability leave please complete the patient sections of your forms and bring them to our office. We reserve the right to charge a $25 fee for this service.
